Our office is accepting new patients. It is possible that you will need a referral from your primary physician, but please check with our office staff to see if your insurance requires that. We would certainly assist with any referrals that would be necessary.
We encourage you to download the Upstate Footcare Patient Forms (see link below). The New Patient Forms include general information questions, and some informational questions regarding your foot problems and past medical history. You will need to print out these forms at home, fill them out, and bring them to your first appointment. Completing the New Patient Information Forms will save you time on your initial visit.
We accept most insurance plans at this time, but please check with our front office staff to see if we do accept your insurance. Insurance verification is recommended prior to your appointment. You should also be aware that Medicaid does not cover podiatry in the state of South Carolina.
